LPS Alternative transportation form: This form is for parents/guardians who do not want their child to be transported by LPS due to COVID concerns. If you use this form please turn this form back into the Athletic Department so that we have it on file. This form needs to be used only if the parent is taking the athlete to the game. If just taking home, parents can just sign out with the coach.
